3 13-May-15Saskatchewan Falls Collaborative3 Aim Aim: To reduce the number of falls by 20%, reduce injuries by 30% by the end of March 2012 Goals: To fully implement a Fall Prevention Strategy at Parkridge Centre as mandated by the Ministry of Health

4 Aim Improvement Objectives: All staff will receive education on fall prevention and the new Fall Prevention Strategy in Sept 2011 All new admissions, including respite clients will have a Scott Fall Risk Screen completed on admission All new admissions who score 7 or greater will have a Care Guide for Fall Prevention completed All current residents will have a Scott Fall Risk Screen completed by Nov 30, 2011 All resident rooms will have a Falls Safety Assessment completed in Oct 2011

6 Results Staff Education Education sessions where held on each neighbourhood over a 2 week period in Sept 2011 Education packages were left on each neighbourhood for staff to read and sign off on

7 Results Scott Fall Risk Screens: (Oct 1-31, 2011) Completed on new admissions: 88% Completed on current residents: 64%

8 Results Fall Prevention Care Guides Completed: (if the Scott Fall Risk Screen was completed and the resident was determined to be high risk) New Admissions: 100% Current Residents: 100%

9 Results Falls Safety Assessments for Resident Rooms: New admissions completed: 92% Current residents completed: 100%

11 13-May-15Saskatchewan Falls Collaborative11 Lessons Learned Implementing a fall prevention strategy in a large facility is time and labour intensive Ongoing education, support and coaching is needed to ensure success “Filling out the form” doesn’t necessarily equate to meaningful care planning and implementation of strategies

12 13-May-15Saskatchewan Falls Collaborative12 Next Steps Continue to perform monthly audits to ensure fall risk assessments and care planning is being completed Continue to provide education and coaching for staff Implement scheduled toileting routines Fully implement hip protector initiative throughout the facility
