1 Case Conferences: Empowering the Person- Centered Treatment Team Amber Hollingsworth, BA Marion County Adult Recovery Team Lead Cummins Behavioral Health Systems, Inc.

2 Case Conference  Simplest Terms: Meeting between consumer, familial & natural supports, and providers - all of which make up the consumer’s Treatment Team  Assumptions:  Consumer is the driving force of treatment  Consumer is a member of their own Treatment Team just as much as their providers  Family and natural supports who have agreed to participate are valuable Treatment Team members as well  All Treatment Team members are equal  Any member of the Treatment Team can request a Case Conference

3 Case Conference: The Purpose  Case Conferences are held in a variety of contexts:  After psychiatric hospitalization to clarify precipitating factors leading to hospitalization and modify services accordingly  Address and problem solve around barriers to treatment (accessibility, transportation, etc.)  Address avoidance behaviors or resistance towards change (exhibited by consumer, family, supports, or all)  Resolve conflict or triangulation among Treatment Team; ensure a unity and transparency  Review progress towards treatment goals and objectives and graduation (discharge) planning  All Case Conferences seek to end in a modification of treatment plan or approach and/or resolution to the issue at-hand which is agreed upon by the Treatment Team

4 Benefits  Consumer empowerment = Greater consumer engagement  Increases solidarity of Treatment Team and unity in recovery progress  Ensures consistency and continuity of approach  Involvement of familial and natural supports increases likelihood of sustained recovery  Helps to ensure intensity of service for high risk/high need consumers
