14. In-Processing (end of June) Documents/Swearing-In: Appointment Affidavit (SF-61) Employment Eligibility Verification (I-9) Pharmacy Prescription Card CPRS Training PIV Badge Schedule with Police. * Must bring two forms ID. * Not required to start July 1 st. Fingerprints expire > 120 days.

19. After In-Processing VA Service coordinates: Pick up PIV Badge Link PIV Badge CPRS Training for electronic medical record Locked doorways (ER, OR, Psych, etc.) Narcotic prescribing
