1 St John Ambulance Community First Responder  Assessment Scenario’s Pack

2 CFR Reassessments  Basic principles of re-assessments, Effective, Safe and Prompt.  All CFR’s will come into the scenario’s with a full CFR training kit and a training AED. Any other items required are listed on each scenario.  Some scenario’s are shown as ‘initial’. These are more appropriate for CFR’s who are completing their first assessment.

3 50 year old with central chest pain. After a time decided by assessor the patient collapses into cardiac arrest. Patient has taken their own GTN (Initial) Skills to be demonstrated  Aspirin administration  Oxygen administration  Consideration of OP airway insertion  Use of pocket mask  Use of AED Equipment Required  Resus Annie  OP Airway manikin  AED trainer  GTN spray  Placebo Aspirin

4 3 month old. Has drowned, in cardiac / respiratory arrest. Skills to be demonstrated  Correct hand placement for paediatric resuscitation  Demonstrate correct PBLS  Consider AED, not using it as patient is under 1yr  Ventilation without pocket mask Equipment Required  Parent  Resus Anne Baby  AED Trainer

5 30 year old. Had a fit, now stopped fitting. Unconscious with a head injury Skills to be demonstrated  Use of recovery position  Use of oxygen  Consider use of an OP airway  Dressing to the wound Equipment Required  Patient  Fake wound

6 40 year old. Fallen after standing on a chair, leg injury, wound visible. GCS 15 throughout Skills to be demonstrated  Wound dressing  Treatment for shock  Consideration of oxygen for shock  Monitor baseline obs, pulse, AVPU, respirations. Equipment Required  Fake wound

7 40 year old. Allergic reaction to nuts. In respiratory distress. Deteriorates into cardiac arrest at assessors discretion. Starts fully conscious enabling oxygen / Epipen administration. Skills to be demonstrated  Patient positioning  Oxygen administration  Consideration of OP airway  AED usage  Correct usage of pocket mask  Epipen Assistance  SJA CFR’s are taught Epipen assist as part of their initial training. Secamb trained CFR’s will not have covered this element so will not be expected to assist. Equipment Required  Training Epipen  Training AED  OP airway manikin  Resus Annie

8 8 year old. Cardiac arrest after a sudden collapse. No history of trauma. Skills to be demonstrated  Correct hand positioning for a paediatric  Correct use of pocket mask  Correct use of AED  Consideration of OP airway Equipment Required  Resus Anne Junior  Training AED  OP airway manikin

9 45 year old. Tripped over an upturned chair. Lying on floor complaining of hip pain and a wound to arm. Skills to be demonstrated  Wound dressing  Immobilise injury using improvised objects  Treatment for shock  Consideration of oxygen for shock  Monitor baseline obs, pulse, AVPU, respirations. Equipment Required  Fake wound  Chair  Blanket

10 45 year old. Known diabetic. Feels lightheaded, sweaty, becoming drowsy. (if the CFR asks, they have done their own BM and it is 2.5mmol) Skills to be demonstrated  Use of glucose gel  Use of recovery position if not treated rapidly, can pass out at assessors discretion.  Consider oxygen therapy  Monitor baseline obs, pulse, AVPU, respirations. Equipment Required  Placebo glucose gel  BM kit (if available)

11 50 year old. Slipped on wet floor, lying on ground with a open wound to arm. Patient then passes out. Skills to be demonstrated  Wound dressing  Recovery position  Consider use of OP airway  Appropriate use of oxygen  Treatment for shock  Monitor baseline obs, pulse, AVPU, respirations. Equipment Required  Fake wound  OP airway manikin

12 50 year old collapsed. In cardiac arrest, bystander CPR ongoing, but bad quality. Skills to be demonstrated  Consider use of OP airway  Appropriate use of oxygen  AED use  Pocket mask use  Correct bystander CPR Equipment Required  Training AED  Resus Annie  OP airway manikin  Bystander
