1. ESY 2010 All paperwork will be online. No ink! No paper!

2. Beginning in March 2010: Clerk will email form from intranet to each responsible person (teacher, nurse, etc.). – ESY Cover sheet (to appropriate teacher) – Student Information Sheet (to appropriate teacher) – Health Services form (if applicable to nurse)

3. Responsible person will complete the form and email back to the clerk. Since teachers do not have access to our ‘s’ drive they must email the form back to the clerk to save into the student’s folder on the ‘s’ drive.

4. The clerk will save the form in the student’s folder created on the ‘S’ drive. To create a student folder go to ‘s’ drive: – All special education personnel – ESY 2010 Find school Create student folder – New folder with Student name – Save

5. Forms to be in student’s folder on ‘S’ drive: Upload from SEAS: – ESY supplement – AU supplement (if AU) – All IEPs for ESY (they should have indicated ESY on the appropriate objectives at ARD) – ACCOMMODATION Plan – BIP (if applicable) – Transportation form (from Additional Documents)

6. Once a student’s folder is complete the clerk will email Janice Ficaro that a student’s ESY folder is complete. Verify all required documents are in student folder by using checklist. Email Janice that student’s ESY folder is complete.
