The proper coding of answer documents is most easily accomplished one test administration at a time by following the detailed and specific instructions provided in your manuals. So How Do I Get Started? Upon receipt, the preprinted information on all answer documents and precoded labels must be verified for accuracy and completeness. Score Code Field The SCORE CODE field in the FOR SCHOOL USE ONLY section indicates whether a student’s test should be scored. Coding of Answer Documents Module Review TF 1. For each subject area on an answer document, only ONE score code should be gridded. 2. On a TAKS-M answer document ARD = the ARD committee has determined that TAKS-M is not the appropriate assessment for the primary SSI test opportunity. 3. The “02” Economic Disadvantage Indicator code (ED) means the tester is not identified as economically disadvantaged. 4. Follow the same steps to grid the TEST TAKEN INFO field even if the student is absent from the test or illness/test irregularity occurs during testing. 5. Failure to mark the appropriate score code may result in a score of ZERO for the student. 6. A voided precoded answer document must be returned with scorable materials. 7. Eligible accommodations must be gridded regardless of whether the student uses them. 8. The “*” score code indicates that the student tested with another answer document for the indicated subject. 9. It is important to verify student demographic and program information. 10. The “SP” grid under Test Taken Info= Special Education.
