2 Optimum Transition Service includes:  Youth empowerment  Family involvement and supports  Academic preparation paired with work experience  !WORK!  Connections to ancillary and post-secondary services (NCWD/Y, 2005)

4 Seamless Transition is…..  A sequential delivery of specific transition service features beginning in early high school  Braiding of resources of transition partners  Resulting in uninterrupted, collaborative transition from public secondary education to employment and/or postsecondary education

5 Flow of Student Services Service Outcomes 10th Grade (or 3 yrs prior to exit) ENROLL Direct Services Self-advocacy instruction Positive personal/career profile DISCOVERY PROCESS Student-led IEP development Family Support/Participation (all services are adjunct to school and academic preparation)

6 Flow of Student Services Service Outcomes 11th Grade (or 2 yrs prior to exit) VR opens case, Applications for post-school services Direct Services Work-based Experiences Student-led IEP development Family Support/Participation (all services are adjunct to school and academic preparation)

10 The Ideal Outcome: Seamless Transition : Youth exits school to post-school settings of choice --with a job, enrolled in postsecondary education, and/or a clear path to adult employment– with supports in place!

11 Selected Outcomes from one demonstration At the Point of Transition:  Has paid employment: 26%  Enrolled in post-secondary ed: 23%  Job and enrolled: 14% TOTAL: 63%  Open cases with VR: 100%  Engaged with CRP: 76%
