People with chronic health conditions often need education, coaching and other interventions to help them gain the confidence, knowledge, skills and motivation to manage the physical, social and emotional impacts of the disease.

Research shows a 90 billion a year cost on productivity and health care costs Patients who are effective in self management have learned the skills has reduced their reliance on primary care
