2 Free Text in Physician Progress Note: Home Oxygen Assessment Face to Face Documentation must be identified as “Face to Face” on MD Progress Note Encounter Date must be included Patients Medical Condition related to the need for Home Oxygen Document: I certify that Home Oxygen is medically Necessary for the Patient due to….I have discussed the need for Home Oxygen with the patient/caregiver

3 Continued Progress Note Must Contain: –Home Oxygen per Nasal Cannula/Mask at 2Liters per Minute continuous/Nocturnally Other Documentation Required to Complete Referral is the Oxygen Saturation or Blood Gas Labs done at Office, Hospital, or Clinical Lab –Refer to Next Slide for Medicare Qualifying Requirements for Home Oxygen Lab Results

4 Home Oxygen Assessment Can not be done using a paper form/template Must be documented in the patient’s medical record
