2  1. This is a supplement to the printed instructions in the manual pages 3-16.  2. Make a copy of your completed questionnaire before sending it to us.  3. PLEASE DO NOT SEND US A RÉSUMÉ.

3 Personnel Questionnaire  Please identify yourself by printing your name in the white box on the lower left side. Last Name, Last Name, First Name, First Name, Middle Initial Middle Initial

4 Personal Page  1. ABPS no longer identifies you by Social Security Number.  2. You may select “Clergy Couple” if your spouse is in ABPS.  3. “Open to Move” makes you available for all searches. “Might Consider” makes you available for about 50% of searches.

5 Personal Page  4. Racial/Ethnic Heritage is used to enhance searches for our diverse American Baptist constituency. You can claim more than one.  5. Where would you like to serve? Choose as many Regions and National groups that you would seriously consider.

6 Address/Contact Page  1. Search Committees have difficulty finding you when your contact information is not UP TO DATE.  2. Your professional credentials are maintained by the Region related to your local membership church.

7 Professional Leadership Status Page  Your leadership status is maintained by the Region. ABPS must verify your status with the Region before completing your Profile.

8 Continuing Education Page  1. ABPS must keep on file a copy of each Continuing Education Certificate. Please indicate on our copy the subject code describing the event (Pages 18-20).  2. Ten contact hours = 1 C.E.U.  3. Do not include Clinical Pastoral Education units here.

9 Education Page  1. ABPS verifies degrees once per year.  2. Please sign Release of Information form for the verification process.  3. Please be sure to give us the official institution name and address.

10 Work History Page  Search committees may question why there are gaps between positions.

11 Preferences for Ministry Page  1. Please be careful to select only types of ministries in which you are willing to serve.  2. Location and congregation size preferences are rarely searched on.

12 Compensation Page  1. Compensation figures are not printed on the Profile but are released to Regional staff with searches.  2. If you have selected Bi-vocational as a Ministry Preference be sure to list compensation as full-time figures.

13 Ministry Specialties Page  1. Your Profile can not be created unless you have chosen eight Ministry Specialties.  2. Specialties will appear on your Profile in code number order not in the order of your preference. 14 Ministry Specialties Page  The experience modifier is a code number representing the experience years NOT the actual number of years. 18 References Page  1. The first reference confirms your relationship to the denomination.  2. Remaining references should be persons that can verbally articulate your ministry skills. These references need not be persons completing a rating form.

20 Narrative Page  1. Remember there is a 3000 character limit including spaces and punctuation.  2. You may submit your Narrative by Email attachment, uploading on our website, or CD/Disk.

21 Rating Forms  1. Evaluators need not be your stated references.  2. Please fill out all information on the cover (including Email).  3. To process your Profile we must have 3 completed forms.

22 Rating Forms  1. Evaluators should know you and your ministry personally.  2. Encourage the evaluators to avoid using “No Basis for Judgment”.
