2009 Salary Bands Pay bands increased using California CPI – $46,000 and under – $46,001 - $92,000 – $92,001 - $137,000 – $137,000 and over

Medical Plans Blue Cross Plans – few/no changes CIGNA – few/no changes HealthNet – support program for metabolic syndrome Kaiser – coinsurance payment for infertility is excluded from out-of-pocket maximum United Behavioral Health – no changes

2009 Staywell Program Must be eligible as of 1/1/09 to receive incentive Incentive (new $$$ amount) – $100 for employee (increased) – $50 for spouse/domestic partner (decreased) Children age 18 and over excluded effective 1/1/09 Health Assessment must be completed by April 15, 2009 to receive incentive

Dental and Vision Delta Dental PPO – Coverage increased for sealants DeltaCare USA – Endodontic coverage added for partial root canal Vision – No plan changes – Retiree plan not open for OE

Health FSA Account used to reimburse eligible out of pocket medical, dental & vision expenses as well as over the counter items Debit card issued – May request additional cards Expenses incurred January 1, 2009 – March 15, 2010 Claim reimbursement by April 15, 2010 (earlier date) Keep all receipts/statements

Additional Benefit Info Mail order provider changing for HealthNet – CVS/Caremark – Effective 12/1/08 – Members notified late October 2008 – Open prescriptions will be transferred (except some controlled drugs) – Members need to register w/CVS/Caremark to use
