1 FASE Application Reviewing Instructions for FASE Committee Members

2 The First Step First, access your application online at http://www.asecho.org/i4a/pages/index.cfm? pageID=3826 http://www.asecho.org/i4a/pages/index.cfm? pageID=3826 Scroll to your name and follow the link to the applications. Each one will open in a window and you may review it or print it to review (some are very long!).

3 The Process Each application will be reviewed by the ASE office to be sure it is complete. A complete application should have the following in place. If they do not, contact Meredith Morovati (mmorovati@asecho.org) immediately:mmorovati@asecho.org – Membership for 2 consecutive years – Membership type that is not a fellow or student (must be out of training) – Completed CV, appropriate boards and certifications are supplied and meet guidelines

4 The Process (continued) Complete applications have the following in place (continued) – Three letters of recommendation are supplied and meet guidelines – Application is complete and paid for – Proof of CME for all members (not researchers/PhDs)

5 The Process (continued) Have the FASE application criteria next to you as you judge each application based first upon the member’s location (US versus International) and then upon the member’s CV and activity. Note that the member’s initial requirements will differ based upon member type (physician, researcher, sonographer, US or international) as indicated on the application.

6 The Process (continued) You will need to determine what activity the member is applying under (education, research, volunteer/leadership or a combination) and look for verification of those activities in the letters of recommendation and CV. If you have a conflict of interest (e.g. work with the applicant) please notify Meredith (mmorovati@asecho.org) so she can reassign the applicant.mmorovati@asecho.org

7 The Process (continued) Each application will be reviewed by two different committee members simultaneously. FASE is awarded to those who receive 2 votes to approve. If you disapprove or have questions, the application will be sent to the Chair, Dr. Ravin Davidoff, for final decision.

8 The Process (continued) Dr. Davidoff has final ruling and may send the application out to the group for input. Disapproved members are given specific information so that they may address the issue and reapply. Each disapproved FASE applicant can reapply one time at no charge.

9 Paperwork You will need to return the first cover of the application with your vote and comments by email or fax. Or, you may email Meredith (mmorovati@asecho.org) with your vote and comments.mmorovati@asecho.org You will be given approximately two weeks to complete your reviews. If you have any questions, scheduling conflicts or other issues contact Meredith Morovati immediately. Comments are required for applicants who receive a disapproval vote.

10 The Process (letters of recommendation) The most common reason for disapproval is that the application material and letters do not specifically attest to the qualifying activity, but instead are more general letters of recommendation. It is insufficient for these letters to reflect activities that are merely part of regular duties. If this is the case, we can contact the letter writers for more information, and the disapproval process will allow the member to address this problem for a reapplication.

11 Notification Approved FASE receive an email from Meredith Morovati. Their FASE status is indicated in ASE’s database at this time. After the email is sent, approved FASE receive a FASE certificate, FASE pin, and letter from FASE Committee Chair. Disapproval notification is sent via post and email.
