2 Learning Objectives This presentation will help you become familiar with various Fund Development forms and reports and learn to use them to help you with your fundraising efforts. 4 Donor and Recognition Forms The TRF Global Contribution Form (123) is the most commonly used form to submit single contributions or recognition requests. 6 Completing Form 123 Provide information about the contribution here. Leave blank if you are using this form just to transfer recognition points.

7 Completing Form 123 Insert the total amount of the cash contribution.

8 Completing Form 123 Specify how the funds are being submitted (check, credit card, etc.)

9 Completing Form 123 Specify how the funds are to be applied: Annual Programs Fund, PolioPlus, etc. (check only one option).

10 Completing Form 123 Insert the information of the person making the gift. If it’s an individual, provide the name and information of that person. If it’s a club, the donor ID is the club number. For a district it’s the district number.

11 Completing Form 123 Specify if you wish to have another individual receive the recognition points that result from this gift, please complete this section. Note: this only applies if the contribution made earns the appropriate amount of points at this time.

12 Completing Form 123 Complete the personal information of the person receiving the recognition credit for this contribution.

13 Completing Form 123 If the transfer of recognition points is enough to make the recipient a PHF or multiple PHF, please complete this field to process the recognition items.

14 Completing Form 123 Complete this section if the contribution is being matched with recognition points. This field specifies who contributes how many points to the person identified in section II of this form.

16 Completing Form 094 Insert the information of the Rotary Club that’s sending the contribution for multiple members.

17 Completing Form 094 Specify the method of the contribution (check, credit card, etc.)

18 Completing Form 094 For credit card contributions, please provide the credit card detail here.

19 Completing Form 094 Specify the designation of the contribution (Annual Programs Fund, PolioPlus, etc.)

20 Completing Form 094 Provide detailed information how the total contribution is to be distributed the the individual club members. Please include their member ID numbers.

23 Completing Form 102 Provide the information of the person completing this form.

24 Completing Form 102 This section authorizes the transfer of the desired number of recognition points. Points can only be awarded to an individual and cannot be moved to the club or district account.

25 Completing Form 102 Specify the amount of recognition points to be transferred.

26 Completing Form 102 Members sign on line “A” to transfer their own recognition points. Add the member ID number of signer on the line below.

27 Completing Form 102 When transferring recognition points from the club account, the club president signs on line “B”.

31 The MCR and CRS Reports The MCR (Monthly Contribution Report) summarizes the Annual, Restricted and Permanent Fund contributions by club in the district.MCR (Monthly Contribution Report) The CRS (Club Recognition Summary) lists the activity for each individual associated with the Rotary Club.CRS (Club Recognition Summary) The DG, DGE, DRFC and Annual Giving Subcommittee Chair as well as Club Presidents and Secretaries can view these reports via Member Access.Member Access Click for more

33 Monthly Contribution Report Membership Count (used to determine per capita amount)

34 Monthly Contribution Report Club Goal (submitted via Club Goal Form 906)

35 Monthly Contribution Report Goal percentage achieved (calculated based on club goal amount submitted)

36 Monthly Contribution Report Per capita amount (calculated based on Annual Giving total for the period and the total number of members listed)

37 Monthly Contribution Report Annual Programs Fund contributions during the most recent month

46 Using the MCR Identify clubs that meet or exceed their goal and recognize their accomplishment.

47 Using the MCR Work with clubs that are short of their goal at the start of the fourth quarter. Promote participation and deadlines.

58 Club Recognition Summary A date is displayed when a cumulative $100 is recorded in Annual Giving for the member in the current Rotary Year.

68 Ordering Foundation materials Foundation brochures, posters and other promotional materials can be ordered at http://shop.rotary.org. In most cases, there is no fee to order and ship. 70 Ordering Foundation materials Select the item you want on the search result page by clicking on the product name. The price will indicate if this is a free publication. Note: our Distribution Center will always send the latest version available (even if the picture may reflect a prior year publication).
