2 VCSE Health Initiatives 2014 Joint commissioning project between the CCG and PH. The Steering Group is made up of PH, CCG and Catalyst £300k of funds from Stockton Borough Council Public Health £333k of funds from Hartlepool & Stockton Clinical Commissioning Group Managed by Catalyst Stockton to facilitate a VCSE response to the challenge of improving health outcomes.

3 Role of the VCSE Innovative projects that deliver specified health outcomes Not the same as what is already in place Collaboration and imagination If smaller then scalable Coherent with future funding plans

4 The Future Health Initiatives but… … tied to Better Care Funding Reducing pressure on current NHS Services Reduced emergency admissions … … and Readmissions

5 The Health Outcomes Achieving Health outcomes: –Healthy Weight –Increased Smoking Cessation –Increased access to psychological therapies (IAPT) & identification of dementia –Reduced social isolation amongst older people –Reduced hospital admissions and readmissions All projects will be underpinned by an ethos of promoting access to health screening services

6 Starting Point VCSE strengths – local, flexible, experienced People & communities we are already in contact with…or know about Organisations already supporting health outcomes including possibly the Health & Wellbeing Fellowship Knowledge of Stockton

7 Size and number of Bids No decision made Encourage small but highly effective bids Also encourage multi-agency bids One project from 2013 has been ringfenced for further funding
