Definition of a Lab Laboratory (Lab) – –A separate, distinct area that has been specifically defined as laboratory space by the Supervisor who is responsible and has control of the activities being performed in that space. Some examples of lab types include: –Hardware –Software –Chemical (Wet) –Power –Testing Size (large or small) does not matter Type (hard walled office, cubicle, etc.) does not matter

EHSMS Coordinators/Committees Rollout to facility/organization Lab Supervisors Designate Lab Manager Ensure training is completed Lab Users Work safely Complete Lab User training Lab Managers Avaya Lab Safety EH&S Program Implementation Instructions (LAB-WS-PI) Ensure compliance with Avaya Lab Safety Worldwide Standard (LAB-WS) and other applicable EH&S regulations Complete Lab Manager training Designated contact for EH&S issues

Lab-WS Requirements Conduct quarterly lab safety inspections, which includes: –Completing the Avaya Quarterly Lab Safety Inspection Checklist (LAB-FM-01) –Ensuring corrective actions are implemented for items found to be “No” not in compliance –Retaining inspection records for a period of three (3) years
