Tertiary Prevention: Specialized Individualized Systems for Students with High-Risk Behavior ~5% Secondary Prevention: Specialized Group Systems for Students with At-Risk Behavior ~15% Primary Prevention: School-/Classroom- Wide Systems for All Students, Staff, & Settings How have we addressed behavior in past all did the same reached general population 80% was for those who could function with these expectations Now addressing the 15 % since have no 5% will include reaching ones who need a bit more not for those who wish to avoid adult contact - shy withdrawn ~80% of Students

PROCEDURES Pick up a SWIM Team Referral Form from the basket in the lounge. Return the completed form to the SWIM team accordion folder in the Nurse’s office. (Top drawer of the file cabinet marked SWIM Team) Please file alphabetically. SWIM Team will meet and discuss student. SWIM Team will notify you of results. Together we will meet with the parents.

Permission for SWIM TEAM Program (Success With Intervention Mentors)
Student_______________________ Grade__________ Dear _______________________________, Congratulations, we would like to include your child in our SWIM TEAM Program for a minimum of nine weeks. A report will be filled out daily by the teachers and checked at the end of the day by a Behavioral Education Program (BEP) Coordinator. Students pick up their report every morning between 9:00 and 9:15 a.m. and then return it between 3:15 and 3:30 p.m. The student will be able to earn incentives and rewards for appropriate behavior. As parents, you are responsible for making sure your child arrives on time each day for check-in and that you review and sign the daily SWIM TEAM Daily Progress Report and return the white copy to the school.
