3. Appraisee: before the appraisal Fill in sections 1 to 12 Cut and paste PDP objectives from 2010-11 into area 5 Upload last years form 4 into area 4 Fill out this years form 3 in area 3 Submit appraisal

4. Appraiser : before appraisal Set correct appraisal date Look through all sections and add comments where appropriate Comment on each CPD item in area 6 (validates CPD credits) Make some notes / print off form3 to aid discussion

5. At appraisal Discuss relevant areas Review last years PDP objectives comment and mark as completed or ongoing Discuss PDP objectives for next year

6. After appraisal: appraisee Write up this years PDP and inform appraiser ( the objectives will be ‘proposed’ ) ? Could be done at appraisal

7. After appraisal: appraiser Convert ‘proposed’ PDP objectives to ‘agreed’ Complete form 4 Sign off appraisal as ‘satisfactory’

8. After appraisal: appraisee Accept appraisal outcome Fill in post appraisal questionnaire Appraisee will then receive PDF file containing all appraisers comments, form 4 and new PDP PCO should receive same document
