2 Important Dates to Remember
Online process begins April 20th ALL employees must complete process by May 8th Benefit changes effective: July 1, June 30, 2016 *Please note that since we pay our health insurance premiums 1 month in advance, the new premiums will come from June’s paycheck.

3 What is Open Enrollment?
Open Enrollment is a once-a-year opportunity to make health insurance election changes Change plans Add or drop coverage for yourself Add or drop coverage for your dependents

4 MID-YEAR QUALIFYING EVENTS What changes can I make outside Open Enrollment?
Eligible qualifying event as defined by IRS guidelines Common examples include: Marriage Divorce Birth or adoption Gain or loss of coverage

Telemedicine New Benefit Anthem BlueCross BlueShield presents ”LiveHealth Online”.  With LiveHealth Online, you get: Immediate doctor visits through live video Your choice of board certified doctors with an average of 15-years of practice Mostly Primary Care Physicians Private, secure and convenient on-line visits How do I get this service? Enroll for free at livehealthonline.com Free on mobile app available How much does it cost? For KeyCare PPO and HealthKeepers HMO members – the copay will be $0 For Lumenos plan members – the copay will be the same as an office visit and subject to the deductible. If you are not on the medical plan, you may use this service as well. The cost per session is $49.00

18 What You Will Need For The Online Process!
Your ePortal username and password Social security number(s) & date(s) of birth for all dependents(s)participating in the benefit plans PCP Code if participating in HMO plan. Click on link to visit anthem.com and “Find a Doctor”.

19 How to Access Open Enrollment through ePortal
From your web browser type eportal.fauquiercounty.gov Once you are logged in Click on Employee Self Service Click Benefits After reviewing your current plans, select the Open Enrollment link

20 Dependent Eligibility Certification
What Do You Need to Do? During Open Enrollment: Review your dependents health care covered by the health plans and make any necessary changes. 2015: If you have not previously submitted proof of eligibility, or if you are adding dependents during Open Enrollment, you will be required to provide proof of eligibility Failure to complete the verification and provide the required documentation will result in the termination of dependent’s coverage.
