Tips and Pearls 1.Read the ENTIRE Continuation Application before you begin. Read carefully Determine sections that apply and don’t apply Note Special Instructions

Tips and Pearls 2.Collect all necessary paperwork and data in one place before starting Participant sign-in sheets List of activities/classes Data submission feedback Budget figures

Tips and Pearls 3.Give your self LOTS of time for reviews and rewrites Don’t wait until the next to the last minute!

Tips and Pearls 3.Follow all instructions and directions for completing the Continuation Application READ CAREFULLY Even if the directions seem redundant and trivial Every section and question has its purpose Pay close attention to Budget/Budget Narrative section

Tips and Pearls 5.Make note of and address any specific directions Dates and numbers request may be specific Attempt to keep to the maximum page requirements

Tips and Pearls 6.Use the “Cut and Paste” buttons judiciously Especially in the INTERIM PROGRESS REPORT and WORK PLAN sections Accurately describe what you have done and what you plan to do whether it is different or the same from previous years

Tips and Pearls 7.DO NOT assume that the reviewer will “read between the lines” “If it is not written down, it didn’t happen”.

Tips and Pearls 8.Provide all requested and required attachments Be sure to label all pages with your Program Name and Grant Number

Tips and Pearls 9.Be as SPECIFIC as possible Especially with examples BUT, remember page limitations OK to use attachments if you need extra space

Tips and Pearls 10.Have at least 1 or 2 other individuals familiar with your program review your Continuation Application before submitting That’s one of the reasons to give yourself extra time Be sure to check for grammar and typo errors

Tips and Pearls Remember, your Continuation Application and the information provided is a legal document that attests what your program has done in the current year and will do in the next year.

Continuation Application 2007- Timeline Timeline for Continuation Application 2007 has been moved up by two months due to anticipated conversion within IHS to the United Financial Management System (UFMS) This system will go live in October 2007 DGO has commented that it should not impact the Demonstration Project grant programs once converted

Continuation Application 2007- Timeline February 15, 2007- Final application package to DGO from DDTP- DONE February 28, 2007- Application available on Grants.gov – available on March 1, 2007 April 30, 2007 – DEADLINE for submission of Continuation Applications 2007

Continuation Application 2007- Timeline continued May 2007- review of applications by DDTP Project Officers and others May 30, 2007- final determinations sent to DGO for acceptable and unacceptable applications* July 30, 2007- Notice of Award will be issued by DGO

Continuation Application 2007 Interim Progress Report Intensive Activities Give number of participants who started and completed each activity Budget Describe problems or challenges with spending or acquiring items for your program

Continuation Application 2007 Work Plan Assessment Tool Submission Not included in the Application Instructions Sent out separately by the Coordinating Center Certify that the Assessment Tool was sent on the Work Plan

Continuation Application-2007 Budget Narrative Personnel Describe who is holding key personnel positions according to the Core Elements Include FTE and In-kind Fringe Benefit List fringe rate for each position listed in personnel Do not lump sum fringe benefit amounts (see sample budget for example)

Continuation Application 2007 Budget Narrative Other Incentives- list types of incentives and specify amount Medications and labs- be specific; list all medications and lab tests Budget Indirect Cost Include a copy of the most current approved cost rate agreement If not current, these funds may be restricted

Continuation Application 2007 Why these changes? Allow for easier review of the Continuation Application For budget, need to be able to determine that costs are “reasonable, allocable and allowable” in order to meet the goals and objectives of the grant for Healthy Heart and Diabetes Prevention Projects.
