Medical for Children For all citizen and immigrant children in the state of Washington Age: up to 19 Family income: Example- Family of 4 can earn up to $ 5,513 per month (2009)

How to Apply Things to remember: No Application is complete without: Proof of Income: 30 days worth of pay stubs Last years taxes Letter from employer stating your monthly income Unemployment statements Letter from client stating how they pay rent, utilities- “the day to day” Proof of Citizenship/Identity: Copies of birth certificate, pass port for child applying If child is over age 16 copies of photo id is needed- school ID, drivers license

Medicaid for Family Adults and children get coverage Adult(s) with child(ren) under age 19 in home Low income & savings Example - Working family of 4, gross monthly income $661 (2009) (50% of earned income is disregarded with this program. So a family of 4 could make up to $1322 before deductions)

Medicaid for Pregnant Women U.S. citizen & immigrant women Family income: Family of 4= $3400 If income is higher, can get coverage & contribute to cost. Similar to a deductible. Coverage through end of pregnancy, plus 2 months.

Medicare Age 65 and older U.S. citizen or permanent residents who have paid into it. Adults & spouses worked in a Medicare covered job for at least 10 years/ 40 quarters. Adults if have an illness/ disability and can’t work any more.
