1 Continuous Quality Improvement Team Structure Level III Level II Level I Level IV Executive Management Team plus the leads from the Districts, CMDP, Hotline and Central Office District 1District 2 D1 D2 D3D4D5D6CMDPHotlineC.O. Level I Teams are at the individual sites. Level II Teams are comprised of the Leads from the Level I meetings plus District management. Districts 1 and 2 have Level III Teams because they are so large. Level III Teams include the Leads from the Level II meetings and Upper District Management. The Level IV Team consists of the Leads from the Level II Teams (Level III for Districts 1&2) and Executive Management.

2 Example CQI Level I Agenda 1.Review Improvement Projects from Previous Level I Meeting a.Determine status of action items previously elevated b.Review progress on local Action Plan 2.Develop New Direct Service Improvement Projects based on Case Record Review Data. For example, improving frequency and quality of visitations or improving investigation response times. 3.Develop New Administrative Improvement Projects on topics such as equipment & training needs or employee morale & retention 4.Summarize meeting and develop consensus on Action Plan Action Plan What ActionBy WhomTimelineOutcome

3 CQI Team Leadership Roles 1.SCRIBE a.Arrange in advance for a meeting place and issue reminders. b.Work with team members to develop the agenda. c.Copy and distribute any materials that team members need for the meeting. d.Have flip chart & marker for brainstorming and solution building. e.Using the CQI General Agenda/Minutes form, take minutes. 2. 4 CQI Team Leadership Roles 3.LEAD a.Give opening remarks at the CQI team meeting, welcoming all and briefly orienting everyone to the process as needed. b.Provide overview of pertinent upper level CQI activities and issues. Focus on issues raised by the team to subsequent levels. c.Support and strengthen team productivity and idea sharing. d.Represent the team at the next level CQI team and effectively present properly documented issues being elevated. e.Assist team members with their work on action plans outside of team meetings.
