If yes, then use of: –Diabetes Management System (DMS), Pharmacy, Laboratory Packages –Approximate lag time in entering PCC forms into RPMS? –If participates in IHS audit, is DMS system used for electronic submission? –If not using RPMS, list patient data system(s) being used. Response rate: Response rate: –66 grantees responded with at least 1 survey from their sites. –Total number of surveys: CVD = 49, DP = 47.
