2 Achievement Completion Success The Passport to Knowledge will be an ongoing assignment throughout the year. At the beginning of the year, each student will make their own passport. The successful completion of each Unit and Unit Test will result in a stamp in their passport. Parents will be required to sign each unit page to acknowledge their child’s completion. At the end of the year a successfully completed passport will have a stamp and parent signature for each unit.

3 This passport is the property of the undersigned and as such shall remain accessible. This passport will be turned over to the teacher on each and every exam day and any other days the teacher asks for it. Failure to do so may result in a deduction of points off the passport assignment. Your signature acts as acknowledgment and understanding of the assignment and what is asked of you. Signature of owner of this passport Last Name, First Name Date of Birth Teacher’s Name Class and Room Number Year You have been given a blue folder and 3 sheets of paper. The folder and paper all need to be cut to 5” by 7” and folded in half across. With the blue folder on the outside, staple all pages together in the middle where you folded. Decorate the front cover with our class seal. On the inside of the front cover, please sign your name. On page one, securely tape a photo of yourself. Neatly print your name, date of birth, teacher’s name, class name and room number, and year.

4 Unit: Nine The Roaring Twenties Parental Signature At the end of each unit, you will take a photograph of a person, place, or object that relates to the unit. Securely tape the photo on the page below the unit. The passport must be brought to class on every test day or points will be deducted from your test. The passport will be turned in to your teacher and will be stamped if you successfully completed the test. Once returned please read any comments your teacher makes. You may also comment on the unit or the test on the same page.

Completion of Unit All units have been successfully completed. Your passport has a stamp for each unit. Most units have been successfully completed. You have stamps for most of your units. One or two stamps may be missing. You may have successfully completed your units; however, more than two stamps are missing. You are missing most or all stamps in your passport. Photograph You have a photograph that correctly reflects each unit. The photographs are appropriate and easy to see and understand. You have a photograph for each unit. One or two may not accurately pertain to the topics. You have a photograph for most units. One or two photographs may be missing. A few don’t pertain to the units. You are missing several photographs or several don’t pertain accurately to the topics learned. Parental Signature You have a parental signature for all units. You received signatures and turned the passport in on time. You have a parental signature for all units. You turned the passport in late once or twice. You are missing one or two signatures. You turned the passport in a few times. You are missing several signatures and turn your passport in late.
