5 Case Report of an Unusual Cause of Severe Dyspnea Case Report of an Unusual Cause of Severe Dyspnea: Diastolic Dysfunction due to Calcific Constrictive Pericarditis Anthony J. Weekes MD, RDMS 1, Joseph Novik MD¹, Department of Emergency Medicine Albert Einstein College of Medicine 1, Montefiore Medical Center, New York, USA Background: 79 y.o. woman with renal failure 5 days severe dyspnea T 98, P 110, BP 120/80, RR 36, SO2 93% Agitated: gasps out words Distended neck veins, poor breath sounds, distant heart sounds, no leg edema Rapid decline leads to intubation Conclusion EM and critical care bedside cardiac ultrasound has focused on LV systolic function and pericardial fluid analysis. Beware of diastolic dysfunction and pericardial disease. Look for diastolic dysfunction by noting muscle relaxation, texture of the muscle and appearance of the pericardium. Pericardiectomy is definitive treatment for this rare case of calcific constrictive pericarditis. Objective: Sonographic Algorithmic Approach to Dyspnea
