4 HSR Plan: 4 Elements “Brief” Project Description Procedures Pertaining to the HSR that will be Followed. Describe in enough detail to be able to evaluate the risks involved. Try to anticipate future changes as far as possible and include them. This must be a separate statement. A reference to a previous application, to a proposal, or to the brief description above will not satisfy this requirement. Potential Risks: Describe and assess any risks (physical, psychological, social, legal, or other) and their likelihood and seriousness. If your methods of research, development, or other activity create potential risks, describe any other methods that you considered and why you did not use them. Informed Consent Form: Attach a copy of the informed consent form you will use. The form must describe in detail any possible risks to the subject, and must state that the subject may withdraw at any time from the project without penalty.

6 B. Specific Procedures to be Followed. Project participants will be given written surveys as part of the overall project assessment. Individuals within the project may also develop surveys for their field projects. These surveys will be used to assess community perceptions of natural hazards and natural resources, and to assess the effectiveness of typical hazard and risk maps for community awareness. These surveys will consist of either written or oral techniques, and will be developed in conjunction with the collaboration of social scientists. In all cases, there will be no physical contact involved in the assessment survey; only paper or oral surveys will be used.

7 C. Potential Risks In all cases, there will be minimum psychological impacts, as the surveys are designed for the reporting of existing behavior and knowledge. The assessments are purely to evaluate either participant or community beliefs and understandings, and do not attempt to make any changes to the beliefs or understandings. Field surveys would be administered by participants actively working in the communities. Surveys will only be taken from willing participants.
