Welcome to the 2015 Prevailing Wage Survey Tutorial This tutorial will assist you in the process of completing Survey Forms. If you still have questions when completed, please call: (304) 558 0481

West Virginia law (§21-5A-5) requires WorkForce West Virginia to determine prevailing wage rates for specified trades and occupations involved in the construction of a public improvement. WorkForce West Virginia, with the assistance of West Virginia University Bureau of Business and Economic Research, and the Center for Business and Economic Research at Marshall University, is conducting a survey to collect relevant data for determining prevailing wage rates in the seven Workforce Investment Areas. The information you provide will be kept strictly confidential and will not be published in any manner allowing identification of your firm. The survey requests that you report non- owner labor and non-residential construction hours performed on the construction site for only the work weeks specified below: Week of August 12, 2014 Week of November 12, 2014 Week of February 12, 2015 Week of May 12, 2015 To complete the survey you will need: Payroll and benefit data for the four weeks listed in the survey The hours worked by employees on non-residential projects The location where the work was completed What type of work employees were performing For electronic filing: If you have more than 10 employees to report, it may be easier to use the Survey Spreadsheet. Your survey packet will contain the survey form and a postage paid envelope to return the survey. Begin by removing these pages from your survey packet.

Complete the required information on this page. What contact information am I asked to provide? Survey ID: Each business receiving a survey will be provided a unique Survey ID. Your Survey ID is located at the top of each page of the survey. Firm: The name by which your company does business. Completed By: The contact person returning the form. Title: The job title or employment section of the person completing the form. Example: Payroll, Owner, Etc. Phone Number: The number we may call to reach the contact person. Email Address: Please provide us with an email address.

You’ll need to determine whether your company was involved in any non-owner labor and non-residential construction work during any of the four weeks identified on the form. Construction includes initial construction, reconstruction, and major renovations. Labor performed on-site in connection with material deliveries is also considered construction work. Routine maintenance (as described on the next slide) is not included except as noted.

If your firm did not do any non-owner labor and non-residential construction during any of these weeks, please check all the boxes that describe why your firm answered no. Check all the boxes that apply and return the forms in the enclosed postage paid envelope.

Next we will assist you in organizing your records to compete the Wage Data Form. Isolate your non-owner labor and non-residential hours performed on the construction site for the work weeks specified below: August 12, 2014 November 12, 2014 February 12, 2015 May 12, 2015 Exclude owners, exempt supervisors who spend 20% or less time performing duties that are manual or physical as opposed to managerial, administrative staff, maintenance work, shop work, any labor not performed on the job-site, and all those who worked on residential job-sites.

We will show you how to report these workers on the Wage Data Form. Be sure you have filled out all the contact information on the form. Locate and refer to the Job Classification Table to determine which employees to report. Identify the occupation in which your employees performed work using the Job Classification Table. Wage Rate Region: This is the region where the work took place. It is not where the business is located. Job Title of Employees: Enter the Code for the job title for all employees listed on the line. If a single employee worked in more than one job title, or in more than one region during the listed work weeks, report the employee on more than one line according to the hours worked. Job Classification Titles, Codes, and Definitions are contained in the Job Classification Booklet

Skill Level: Put a check in the column that best describes the skill level of the Employee(s) on the line Skilled Craftsman/Journeyman: A fully skilled craftsman who can work independently in the trade or job title. Generally, a skilled craftsman has a minimum of four years of verifiable trade-specific experience or has completed a state certified apprenticeship program in the applicable trade. Skilled Craftsman/Journeyman are licensed where licensing is required. Entry-level Craftsman/Sub-Journeyman: A worker who cannot work independently in the trade or job title. Generally, an Entry-level Craftsman/Sub-Journeyman has fewer than four years of trade-specific experience and has not completed a state certified apprenticeship program in the applicable trade. Apprentice: You are not required to submit information for the Registered Apprentices. Basic Hourly Rate: Hourly wage rate Exclude: ***Include only the workers’ BASE RATE*** Fringe Benefits including vacation pay Shift differentials Overtime Pay Per diem Zone pay Differential for being a crew leader, site supervisor, or non-exempt supervisor

Fringe Benefit Reporting for Prevailing Wage Projects: Health & Welfare: Monthly premium, adjusted hourly rate, or annual rate for employee. Pension: Employer monthly contribution amount. Please enter adjusted hourly rate, monthly rate, annual rate, or percentage. Holiday & Vacation: Number of paid days off employee receives each year. Apprentice Training: Employer contribution amount. Please indicate if the number is an Adjusted Hourly Rate, Monthly Rate, or Annual Rate. Other Benefits: Cash paid in lieu of fringe benefits should be reported here. DO NOT include: drug testing, industry funds, trade promo funds, safety training, travel pay per diem, equipment costs or workers’ compensation. Please enter the PWR required fringe, monthly rate, or annual rate. Note: If an employee was paid more than one rate of pay, or worked in more than one occupation or region, you need to provide the hours and wages for each case. When this occurs, you will need to use more than one line on the survey form for that employee.

Fringe Benefit Reporting for Prevailing Wage Projects: When working on Prevailing Wage Projects, there is a base wage rate and a Fringe Benefit Rate. PLEASE SEPARATE THE TWO RATES. If your firm pays higher than the Prevailing Wage, please disregard this example and report the amount paid as if it were not a PWR project. Report the base wage in the Report the total Fringe Benefit “Base Hourly Rate” Column Rate in the “Other Benefits” column

Number of Workers: If more than one employee worked in the same region, has the same pay rate, skill level, and all of the same fringe benefits, the total number of employees may be entered.
