2 Southern Health services in Fareham and Gosport Community nurses Enhanced Recovery and Support at Home Integrated Care Teams Inpatient beds at Gosport War Memorial Hospital Specialist services such as Parkinson’s Disease, falls prevention, Multiple Sclerosis, palliative care, heart failure, respiratory, home oxygen, continence, tissue viability, pulmonary rehabilitation

3 Community Nurses Provide care for housebound patients with a medical needs, such as: Patients with a chronic, complex condition Patients who are housebound due to a surgical operation and/or physical health condition End-Of-Life care Referral routes: Self referral GPs Acute hospitals Carers Social Services

4 Two-Hour Rapid Response Service For patients with complex needs who become extremely unwell, but can be cared for in their own homes Look at care holistically and provide support for carers as well as patients Work with voluntary sector, Social Services, Physiotherapists, Occupational Therapists, hospices Referral via GPs Twilight Service Nursing care provided in patients’ homes between 5pm and 11:30pm Night Nursing Service Predominantly support for End-Of-Life patients who are extremely unwell Provide care between 10pm and 7am

5 Enhanced Support and Recovery at Home Helps older patients with complex health and social care needs in the Fareham and Gosport area Support people to live independently after hospital stay Provides care people need to avoid them being admitted in the first place Focus on ‘three Rs’ – recovery, reablement and rehabilitation Specialist team of nurses, Physiotherapists, Occupational Therapists, Social Services Intensive support, tailored to individual Support and confidence giving Helped more than 400 people since launch in October Visits up to four times a day for up to four weeks

6 Inpatient beds : Gosport War Memorial Hospital Sultan ward Older patients Referrals via GP from home, Intravenous antibiotics, diuretics Short-stay rehabilitation for patients transferred from acute hospitals Daedalus ward and Dryad ward Holistic care for older people with acute mental health needs unable to look after themselves or be looked after by a relative or carer Majority of patients have memory needs and may experience difficulties with daily life

7 Parkinson’s Disease service Monthly nurse-led clinic at Gosport War Memorial Hospital GP clinics Specialist nurse assessment to look at needs of patients, families and carers Support to monitor and review effectiveness of medication Partnership working with others involved in caring for a person with Parkinson’s Education to individuals, groups, health and social care professionals to promote understanding and raise awareness Telephone and email access to specialist nurses Self referral with GP/consultant referral letter, or referral via GP, consultant or any Multi-Disciplinary Healthcare Professional

8 Services for people with Multiple Sclerosis Education for adults with confirmed diagnosis of MS Specialist nurses can provide home visits Clinics at Gosport War Memorial Hospital and The Rainbow Centre, Fareham Monthly mother and toddler group Advice on managing tiredness and other symptoms Help to get equipment to assist with everyday tasks Help to develop routines to make day-to-day life easier Events for people newly diagnosed with MS twice a year Education for other health professionals to improve awareness and understanding of MS Referral via GP or consultant

9 Diabetes services Help empower adults to manage diabetes through education, support and training Team includes diabetes specialist nurses, consultants and specialist dieticians ‘DESMOND’ days for patients – part of national programme of self management and education for people with, or at risk, of Type 2 Diabetes Once diagnosed patients will receive details of how to take part from their healthcare professional Support patients, their families and carers by providing education and guidance on managing diabetes. Education and clinical support to healthcare professionals, social services, care homes and anyone caring for people with diabetes Work with local diabetes organisations
