Over 20 years of Cardiovascular Nursing Leadership Experience 16 Years with Hospital Corporation of America (HCA) in Florida and Colorado 4 Years with Centura Health At Stanford, recently led a cross-functional team in a two year initiative in “Reducing Avoidable Readmissions for Heart Failure” Team uses Lean Methodology- developed processes and tools to implement evidence based “intervention bundles” and developed methods to prospectively identify patients in the electronic health record, and created workflows for physicians, nurses, pharmacists and case managers to support the heart failure patients in the quality of their transitions of care as they move through a complex organization
