BOARD OF GOVERNORS OF THE FEDERAL RESERVE SYSTEM 7/2/20153 Affiliate Model If an organization already has EUAC established for the bank: Need to set up new or make changes to existing FedLine® Subscribers to grant access to Reporting Central (step 3). AND Need to send in Subscriber Access Request Form (step 4). For organizations that have an ABA or are utilizing an ABA through the affiliate model, the next two slides will provide high level overview for Steps 1-3. For organizations with a CIN or ONLY an RSSD-ID and are not utilizing an ABA through the affiliate model, call the Customer Contact Center at (888) 333-7010, option 2 for assistance.

Designate Authorized Individuals Submit a Board Resolution and Official Authorization List (OAL)Board Resolution and Official Authorization List These forms can be found on FRBServices.org by selecting: Forms > Account Services > Account Holder Board Resolution and Official Authorization List Step 2. Designate End User Authorization Contacts (EUACs) All organizations need to complete a separate Federal Reserve Bank End User Authorization Contact (EUAC) Form for each individual identified as needing FedLine EUAC authority.Federal Reserve Bank End User Authorization Contact (EUAC) Form

Set up new or makes changes to existing FedLine Subscribers Request FedLine Web service access for the individuals (Subscribers) who will be using the Reporting Central application. Authorized EUACs will submit a Subscriber request via the EUAC Center within FedLine Home for each individual identified as needing Subscriber access to Reporting Central.

BOARD OF GOVERNORS OF THE FEDERAL RESERVE SYSTEM 7/2/20156 Federal Reserve Bank Subscriber Access Request Form (Form RC-1) Step 4: Enroll Subscribers to specific reports If a Subscriber already has a token, an EUAC only needs to set up existing Subscribers with Reporting Central and complete this form to grant access to reports in Reporting Central. EUAC cannot sign himself/herself up as a Subscriber. A different EUAC must sign that person up as a Subscriber.

Reporting Institution’s Legal Entity Name - Name of institution that submits the report (bank or holding company). Enter only one name. RSSD ID – RSSD ID of entity (bank or holding company). Enter only one RSSD ID. Address – Physical address of institution. Telephone Number – Subscriber’s work phone number. Work Email Address – Subscriber’s work email address.

For example, a bank may select a FR 2900 report (either FR 2900 or FR 2900 Q) AND FR Y-8. FR Y-8 is selected under the bank’s RSSD ID. For example, a holding company may select FR Y-9 C AND FR Y-9 LP. More than one report can be selected, but it has to be filed for the institution listed above. Reports may be selected even though they are not currently available in Reporting Central. IESUB will continue to be used until the report is migrated to Reporting Central.

Once each Subscriber request has been processed by the Customer Contact Center, an EUAC will be able to click “Request Reporting Series Access” within the EUAC Center to access a pre-populated Form RC-1 for each credentialed Subscriber.online EUAC emails completed and signed form(s) to mpls.statistics@mpls.frb.org. mpls.statistics@mpls.frb.org **Email must come from EUAC** Fax the form(s) to (612) 204-5905 Mail the form(s) to: Federal Reserve Bank of Minneapolis P.O.
