6. A shift towards prevention No Concerns identified at 27 Month Child Health Review

7. Aim of the Improvement Work To maximise global development and wellbeing outcomes for children at 27-30 months of age by earlier detection and prevention of areas of concern. Aim 95% of children aged 12 -15 months of the test population designated by the Health Visitor caseload in the Coatbridge locality will have a review completed by 31 March 2015 and provided with interventions around areas of concern and anticipatory guidance to prevent areas of concern developing at a later stage.

Have been able to rapidly apply learning from tests of change Developed their own parent feedback form and anticipatory guidance checklist based on their learning while carrying out the test. We have a passion and want to get it right for every child (GIRFEC), were in a team and can respond as a team. We have drive and ambition to make things better.

11. Anticipatory Guidance The team started using the checklist on 05/09/2014. The feedback on the checklist has been very positive from the team. They have found it very useful, especially when filling out the wellbeing assessment.

Q3 I would recommend this contact with a health visitor be available to other parents? 21 Parental Review Questionnaires received as of 07/10/14. All have answered Yes so far to these questions. We are learning that parents value the strengthening relationship with health visitor and reassurance given, not just the assessment itself.

22. + Comments by Parents After my first baby didn’t have any further assessment then just before school found out she needed speech therapy. I think early assessment like this is excellent for picking up any issues. Very informative, particularly having older child with autism. Good to know my child is doing well.
