What information is reported on the 1095-B? Name and TIN (or DOB) of every individual covered under the plan – DOB may be used only after reporting entity asks for a TIN 3 times Employer information for employer sponsored coverage Coverage provider information (health plan) Month(s) of coverage for each covered individual – One-day rule: if you are enrolled in MEC for one day of a month you have MEC for that month 4

What information is reported on the 1095-C? Employee general information Employer general information Employee offer and coverage – Information on the health care coverage offered to full-time employees. This will be used by the IRS in determining an employee’s eligibility for the premium tax credit and to administer section 4980H (employer’s shared responsibility) – Indicator codes will be used to report certain information 6
