Supervisors & other Team Members: Dr Ian Gooding, Dr David Gannon, Ms Mandy Winslet, Ms Yvonne Goodchild Effect of this QIP on Clinical Practice and Patient Safety:  Standardization of weekend handover of medical ward patients  More accurate and permanent record of weekend ward tasks increasing accountability and tracking  Handover process more time efficient and informative  More appropriate delegation of tasks  Permitting on call team particularly medical SpR safer management of ward patients during the weekend Next Steps:  To re-educate junior doctors on the method and the importance of recording task completion  Pilot a Consultant-led Friday afternoon face-to-face weekend handover meeting Recommendations: Successful implementation of a new process in hospital medicine requires comprehensives assessment of users’ needs and effective communication, education and training.
