Part B Indicator 13 For the FFY 09 Submission due 2/01/11: A new Indicator this year (FFY 2009) Report using the SPP template An optional APR template is also available Provide baseline data (2009-10) Provide targets and develop improvement activities for three years (FFY 2010, 2011, 2012), with stakeholder input Targets must be 100%

Indicator 13 Changes to the indictor Revised indicator language IEP transition planning now includes eight required elements for all students 16 and older Evidence of each in the IEP now required for individual compliance Western Regional Resource Center APR Clinic 2010

Indicator 13 Eight Required Elements 1.Appropriate measurable postsecondary goals that cover education or training, employment, and, as needed, independent living 2.Updated annually 3.Based on age-appropriate transition assessment 4.Transition services in the IEP that will reasonably enable the student to meet his or her postsecondary goals 5.Transition services include courses of study that will reasonably enable the student to meet postsecondary goals 6.Annual IEP goal related to the student’s transition services needs 7.Evidence the student was invited to the IEP Team meeting where transition services were discussed 8.If appropriate, evidence a representative of any participating agency was invited to the IEP meeting with the prior consent (parent or student who has reached the age of majority)

Indicator 13 (by SPP section) Overview of SPP Development Describe how ‘broad input’ from stakeholders was obtained for I 13 SPP revisions, including identification of improvement activities Could mention this clinic among activities Also describe how the Revised SPP/APR will be disseminated to the public (post on website, etc.) This information can be provided by indicator or summarized at front of document

Indicator 13 Overview of Issue/Description of System or Process Describe your B13 data collection process (i.e. by data base or monitoring) Describe procedures used to collect data (e.g. how LEAs were selected for monitoring, etc.) Describe any checklists used & other methods in place to ensure data accuracy (inter-rater reliability) Describe what the State did last year for B13 & any impact on assuring all LEAs are included during SPP cycle (if applicable)

Indicator 13 Baseline Data for FFY 2009 (2009 – 2010) Provide actual numbers used for calculations Clearly show calculations and describe baseline data Provide any additional breakdowns of these data that are relevant to your discussions of baseline data and current or proposed improvement activities

Indicator 13 Discussion of baseline data How does baseline compare to required target of 100% and minimum threshold of 75%? Describe any data collection/analysis issues and implications for identified or proposed improvement activities Discuss other current or planned improvement activities that may impact future results Describe any alignment of improvement activities with other indicators (i.e. 1, 2, 14) that may impact future performance

Indictor 13 Correction of Previously Identified Noncompliance (if applicable) Can use optional B13 APR template to discuss any remaining FFY 07 findings of noncompliance Describe specific actions taken to verify correction of any remaining FFY 07 findings, consistent with OSEP memo 09-02 Any FFY 08 findings of noncompliance should be reported in the FFY 09 APR under B15

Indictor 13 Measurable and Rigorous Targets 100% Three years: FFY 2010, 2011, 2012 Improvement Activities/Timelines/Resources Should be clear, measurable and related to the issues identified in the data discussion May be continuing activities and/or new Can also organize by type or across other indicators
