Multiple visits to GP Multiple visits to Health Visitor Families receive conflicting advice Referrals to a variety of secondary care service Frequent ED/urgent care attendances- avoidable

More than just another clinic… GPsI – liaise with local GPs, lead on education and training sessions for GPs Community nurse- school/nursery liaison, home visits Dietician –training for community childrens dieticians Health visitor education

Proforma Community service - conditions that need initial GP diagnosis but then can be discharged/nurse and dietician FU. Secondary care service- complex cases

1 community clinic/week- GP, nurse, dietician. 6-7 new patients per clinic Nurse and dietician follow-up clinic Children may move from community to hospital service and vice versa depending on progression of disease
