The Paramedic Referral process was conceived and developed to directly support Ontario’s Action Plan for Health Care – Living Longer Living Well (2012). The tool kit provides a best practice framework for community referrals. The toolkit was been developed with the generous support of the Ontario Ministry of Health and Long Term Care Implementation Branch. Toolkit designed to ensure Paramedic services could implement with 0 – little cost and workload.

Included in the toolkit are the resources to support and implement best practices for community referrals made by Paramedic Services. Resources include the following evidence based components: Supporting Research for Community Paramedic Programs Education and Training Clinical Prediction Rule (PERIL - Paramedics assessing Elders at Risk for Independence Loss ) Standardized Referral Form and Process Sample Pamphlets and Information Electronic Referral

1350 Certificates issued by Premergency for completion of training program.

Use of form for referrals to Service CP Programs, Mental Health, Addiction Services or other Community Support Agencies Referral to be embedded directly into Client Health Record at CCAC Bidirectional feedback with CCAC Further Research into effectiveness of PERIL screening.
