For instance, a photo may show violations of Subpart M (Fall Protection), Subpart X (Ladders), and Subpart L (Scaffolds). Road Workers in the medium of a Florida 2 lane road with out any MOT protection picking up trash. Notice first worker’s traffic vest not properly worn. Also notice in side mirror of vehicle and you will see workers truck parked over 300 feet away from them.

On 5/5/2011, worker in Key Biscayne, Florida while unloading a truck died when the cargo of large windows that were being shipped moved and crushed him between the side wall of the trailer and the cargo.

Two workers use their heads to get enough height in order to complete their repairs. The Scaffold use is not only not properly assembled, but also not properly planked. In addition the worker on the ladder is not properly supported and of course the ladder should not be on and over the scaffold.

Is this worker higher than 6 ft? Do you think he requires any type of fall protection? If so what kind? What would you do? No Harness, landyard, no anchor point.

Not only is this scaffold not properly assembled i. e
Not only is this scaffold not properly assembled i.e. no planks, mud seals, unlevel surface and over a hole. The worker also is going to cross over a large hole in the ground with no protection to prevent his fall. Are you wondering where he is going in a hurry?

The worker crossed the unprotected trench in order to use the bathroom which apparently on this site is the hole next to the wall.
