Developing a Wellness Recovery Action Plan Transitioning back into Society

Saadiqah Morgan , CPS (WRAP Facilitator) Creating Increased Connections 18 S. 4th Street, Bridgeport, PA 19405 Ex-204 Wrap was designed with seven components to help a person stay well and to also have action plans in place when going through a crisis.

Key Recovery Concepts What are the things people Hope for getting out?
How do we get people coming home to take Personal Responsibility? Where does the Education come in? What does Self–Advocacy look like? How do we help them with Supports?

Wellness Toolbox for a warm hand off into society
This is a list of tools that are most commonly used to stay well and to help relieve symptoms. Options Jobs Stable housing Food access to internet Families Examples: see counselor, attend support group, take medications, extra rest, do normal things like wash hair or take a bath.

Daily Maintenance List
Coming back into Society it is important to consider everything you need to do on a daily basis to not only support an individual with the transition into society. It is equally important to have these things set up to maintain and stay in Society! Daily Maintenance plan has three list, 1. What you are like when you are well. 2. Things you need to do to keep yourself well. 3. Things you might do to keep yourself well.

Triggers & Action Plans for Society
*Now this is where you really work with the individual coming home. *We know most people are subject to have to go back into the environment they came from. EAMPLE: Local bars or places where you got high, family friction, anniversary dates of losses or trauma

Early Warning Signs These are things we need to discuss with individuals one on one basis. Early warning signs can also be worked on when you are helping the individual explore triggers. Examples:butterflys in stomach, anxiety, lack of motivation,increased smoking,not keeping appointments ACTION PLAN: Do the things that are on my daily maintenance plan weather I feel like it or not.

Things Are Breaking Down & Action Plan in Society
In spite of our best efforts, our symptoms may progress to the point where they are very uncomfortable, serious and even dangerous, but we are still able to take some action on our own behalf. This is a very important time. What are some of the options people would have? Examples: Headaches, sleeping all the time, not eating, negative thoughts Action Plans:contact doctor, take some kind of action so I am not alone to hurt myself give personal things to supporter such as car keys medications, credit cards etc.
