Types of Individualized Health Care Plans for Students with Type 1 Diabetes O IHPs – Individualized Health Plans O Emergency Action Plans O Lock Downs O Weather Emergencies O Evacuations KBN 2014

IHPs (Individualized Health Plans) O The planning phase must include the parent, student, teachers/other school personnel, school nurse, and health care provider. O Must rely heavily on info provided through the DMMP (Diabetes Medical Management Plan). O Must address all phases of the school day, including bus transportation, field trips, specials classes ( PE, music, art, etc.), any classroom and education accommodations, and extra curricular activities. IHPs PLAN OF ACTION MUST BE SPECIFICALLY SPELLED OUT FOR : O HYPOGLYCEMIA ( LOWS) O HYPERGLYCEMIA (HIGHS) O Diabetic Ketoacidosis (DKA) INSTRUCTIONS FOR SYMPTOMS & TREATMENT KBN 2014

IHPs An IHP can also include ACTION PLANS FOR: O LOCK DOWN SITUATIONS O SEVERE WEATHER EMERGENCIES O SCHOOL BUILDING EVACUATIONS KBN 2014

Lock Downs O In a Lock Down – the student will be confined to the classroom, or the building. O Actions to plan for during a Lock Down: How will the student access food? How will the student access the glucometer? How will the student access the insulin? KBN 2014

Lock Downs O If the student is confined to a classroom – is the school employee in that classroom trained on how to handle the needs of the student with diabetes? O How long might the Lock Down continue? O Do you as the designated health care individual in that building have a means of communication with the classroom where the student is confined? O Are there extra supplies in the student’s kit, backpack, or classroom? KBN 2014

WEATHER EMERGENCIES The same questions will arise for weather emergencies O How will the student access the needed supplies, medication, and food during a weather emergency? O The age of the student must be considered in planning for weather emergencies. O Does the student carry a diabetes care kit, or back pack? KBN 2014

IHPs O Develop emergency plan and communicate to school staff, parent, and health care provider. O Collaborate on any needed changes to the IHP throughout the school year. O The IHP is effective only for the current school year. O A new IHP, specific to the student must be developed each year. KBN 2014

IHPs O A copy of the IHP is available to school staff on a “need to know” basis. O Confidentiality of the health information is to be maintained at all times. O Provide the necessary training for all school staff regarding the DMMP and IHP plan for care. O Obtain release of medical information permission form. KBN 2014
