1 How to Chart a Medical Records Request in the PHI Log
Step 1: From the NextGen Applications Launcher, Log into EHR. Step 2: Enter NextGen username and password.

Step 3: From the top toolbar, click the Patient Lookup icon to search a patient’s name. Enter the last name, first name, and date of birth. Click “Find”.

Step 4: Double click on the patient’s name to open chart. Note: Only select a name that has a yellow chart icon to the left of the name, as the blue globe icon represents a name that currently does not have a chart created.

Step 5: From the History Bar, click the “New” button to create a new encounter. Note: Make sure the “Four Point Check” is done prior to clicking the “New” button.

Step 6: From the History Bar, click the “Templates” module. A small “Select Template” box will appear on the screen. Step 7: Switch the Medical Records tab, to Demographics, then click “PHI Log” from the items in the list. Click the “OK” button.

Step 8: Double click in the grid to open the PHI Log. Grid will turn blue during the process. Note: If there is a previous record appearing in the grid, double click on the text in the grid to open the PHI log.

Step 9: Complete all relevant sections of the form. Click “Save”, then click “Close”. Note: If a request was completed previously, click “Clear for add” to save the previous request this also clears the fields to start a new request.

After clicking “Save”, then “close” from the previous screen, information will appear in the grid of the PHI Log. Note: If there is no information populated into the grid, the end user may not have clicked “save” before clicking “close”.
