22 Wellness Features Retiring o Liveon Health Portal replaces Personal Health Manager –No historical data will transfer to new wellness portal. Members who wish to print or save information will need to visit the site before 12-31-2012 o Expanded Health Assessment integrated with Liveon portal –No historical data will transfer to new tool. Members who wish to print reports or certificates of completion will need to do so by midnight on 12-31-2012 o Life Points replaces Blue Points SM program –Valid Blue Points balances as of 1-1-2013 will migrate to the Life Points portal within 1Q 2013 What will not migrate? Kids' Stamp program (ages 6-12) and Teen Points program (ages 13-17) must be redeemed by midnight on 12-31-2012 Blue Points history and detailed information for members 18+ will no longer be available after 12-31-2012 22
