2013 ANNUAL REGISTRATION OF STUDENTS WITH VISUAL IMPAIRMENTS AND DEAFBLIND ANNUAL CENSUS WHO QUALIFIES FOR THE DEAFBLIND CENSUS It is the policy of Region 10 Education Service Center not to discriminate on the basis of race, color, national origin, sex or handicap in its vocational programs, services or activities as required by Title VI of the Civil Rights Act of 1964, as amended; Title IX of the Education Amendments of 1972; and Section 503 and 504 of the Rehabilitation Act of 1973, as amended. Region 10 Education Service Center will take steps to ensure that lack of English language skills will not be a barrier to admission and participation in all educational programs and services.

Other Considerations If a student is coded Deafblind (DB), and has a visual impairment (VI) code, then the VI code does not need to be listed If a student is coded Deafblind (DB) and receives services from a Regional Day School Program for the Deaf, then auditory impairment (AI) code must be listed Deafblind (DB) should be listed first in order of PEIMS codes

Consent for Release of Confidential Information All students with a visual impairment must have Consent for Release of Confidential Information from the Registration of Students with Visual Impairments signed In addition, all students on the Deafblind Census must have Consent for Release of Confidential Information from the Deafblind Census signed Students who are coded VI and also on the Deafblind Census must have both forms in their eligibility folders http://ritter.tea.state.tx.us/special.ed/forms/viforms.html
