Field Trips Please note: Before any field trip can be covered by Mattole Valley Charter School, the IST needs to submit; field trip request form, and a field trip drivers form (for those transporting any student other than their own).

Forms We have several forms and one checklist that are used for field trips. Please take the time to look at these prior to planning an event. Field Trip Request Form This is the form that needs to come to the office first(707)476- 8069 Please fill out completely. To allow time for processing; day trips allow about a week, for overnight trips allow at least a month maybe more. These type of field trips must be approved by the school board which meets only once monthly

Day Trips Once you have received your authorization back, to go on your day trip, you need to make sure you have (and take with you) the Parent Permission slips. These are not sent to the charter office. Also a list of chaperones (must be at least 21 of age). One chaperone per six students ratio. Contact person with phone number. Before departure, fax the actual list of students and chaperones attending to (707)476-8069. Have a great and safe time!

Parent Permission Form The Parent Permission Form needs to be filled out before a student can go on any field trip. As the teacher/IST you must retain this form while you are on the field trip in case of an emergency. You do not send copies of this document to the charter office.

Overnight Field Trips Before submitting for an overnight field trip, make sure you have the following; Complete Field Trip Request Form Complete Field Trip Agenda; includes times and locations, plus contact phone numbers. List of accompanying chaperones All attending students Excursion/Waiver Forms, filled out completely. Incomplete forms will be sent back! Driver Forms for each driver transporting any student other than their own family member. Assumption of Risk If your packet is missing any one part of the requested materials, the school board will not consider it for approval. Plan ahead and gather all the information needed. Fax all materials to (707)476-8069.

Overnight Field Trip Form This very important form is for Overnight or specific activities that are deemed hazardous. Please be sure to have your students and parents fill out the form completely, including all necessary signatures. Please DO send copies to the charter office (707)476-8069. This must be included as part of your Overnight packet or your field trip will not be approved.

Field Trip Driver Form Every school year we need to have on file driver’s forms for those individuals who will be driving other students other than their own. All sections of this form need to be filled out along with a copy of current CA driver’s license and current insurance card. Please fax this information to (707)476-8069.

Field Trip Check List The Field trip check list is to help teachers understand what paperwork needs to go where, before they leave for a field trip, whether it be for a day trip or overnight. A handy tool. Please use it!

Assumption of Risk Form This very important form is for specific activities that are deemed hazardous. Field Trip Tag-a-longs This is just a read through document for those who are not part of the school (siblings, etc.) Parents who volunteer, not just to be there for their child, actively helping with the event need to be processed as any other volunteer for the school. This includes: application process, screening, sign in/ of campus. The district decides how many volunteers are applicable for a given field trip. Siblings (non-district enrolled minors) are not allowed to participate in district field trips. For non-volunteer parent/guardians at a public venue, cannot: participate in district transportation, must arrive separately, their child under the supervision of the district cannot go off with their parent. If they do, the parent signs their child out, takes custody of their child and the field trip has ended for that student.
