Deductible credit of $250.00 for Employee and Spouse when the online Health Assessment is completed between July 2013-December 2014. Waive Health Care Insurance OSU provides health care insurance to all eligible employees May waive coverage for other group coverage or for religious reasons OSU will contribute $200 a month to a flexible spending or dependent care account Form must be submitted each year Two-part process on Web for Employees

Flexible Spending Account (FSA) Healthcare FSA – Current participants need to re-enroll for 2014 – Pre-funded account administered by WageWorks – Receive a WageWorks debit card – $2,500 annual maximum employee contribution

Dependent Care Spending Account DCA is separate from FSA Eligible Children under the age of 13 Before and after school care is acceptable $5000 annual maximum – $2500 per parent if both are contributing
