1. ENROLLMENT

2. Confirmation of presence on enrollment day o Before 31.8.2015 I will confirm if I will come or not to enroll in Student Information System o I will visit this page https://is.cuni.cz/studium/eng/login.php?do=uchahttps://is.cuni.cz/studium/eng/login.php?do=ucha o Login - Admission procedure status - I will put my e-mail and password o I will see my application and I will click on it o There will be my number of points from the test, final result “Admitted“ and details to the enrollment day o “Please fill in following form to help the university” - I will fill that table and Faculty will be able to see if I will come or not

3. When and where o 18.09.2015 o Start at 10 am (registration is at 9.15am) o Dean’s office, Kateřinská 32, Prague 2 (Stomatology lecture hall, 2 nd floor, door number 3.033) o The enrolment takes usually 4-5 hours, please don’t forget to bring something to drink and snack

4. What I should bring to the enrollment? o Confirmation about payment o Passport (ID) and the COPY of your valid passport(ID) on a paper o 3 pictures which will have PASSPORT SIZE o provide a notarized and translated copy of high school diploma (by Czech notary and to the Czech language)

5. Accommodation o We have the accommodation guaranteed for you, if you will require it o You have to let us know in advance (few months before enrollment day) and write us the exact day when do you want to come to Prague o Accommodation is in the Halls of Residence of Charles University in Prague o Usually studios are consisting of 2 double-bed rooms and common kitchen, bathroom and WC o Single rooms are not usually available o Price: around 9 EUR a day o Information and pictures http://kam.cuni.cz/KAM-47.html http://kam.cuni.cz/KAM-47.html o Our students are usually in Kolej Hvezda, Kajetanka and Komenskeho

6. 7. Matriculation ceremony o 21.10.2015 o ATTENDANCE IS COMPULSORY o The official festive matriculation will be held in the big auditorium of Karolinum (Ovocný trh 3, Prague 1) o Please come in formal dress (don’t forget to pack it at home) o Teaching that day will be cancelled

8. If you won’t be able to come on enrollment o There must be some serious reason for not to come on enrollment (like illness etc.) o The maximum allowed time for starting to study on our faculty in particular academic year is 14 days o After this time it won’t possible to enrol to our faculty o Please let us know in advance if you will have any trouble to come to enroll
