U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration How to Submit Reports All operators or their agents will have to register through the PHMSA portal via the ODES site http://opsweb.phmsa.dot.gov http://opsweb.phmsa.dot.gov You will need to know your OPID and ODES PIN to register in the Portal If you do not have an OPID and PIN or you cannot remember it, please go to http://opsweb.phmsa.dot.gov http://opsweb.phmsa.dot.gov The PHMSA Portal will be used to access all 2010 Annual Report forms and the Mechanical Fitting Failure Form. Click on “create account” - 17 -

Once the combination matches, your userid will allow you to submit reports for that Operator ID. This process will be repeated to associate addition OPIDs to your account. - 18 -
