17 U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration How to Submit Reports All operators or their agents will have to register through the PHMSA portal via the ODES site http://opsweb.phmsa.dot.gov http://opsweb.phmsa.dot.gov You will need to know your OPID and ODES PIN to register in the Portal If you do not have an OPID and PIN or you cannot remember it, please go to http://opsweb.phmsa.dot.gov http://opsweb.phmsa.dot.gov The PHMSA Portal will be used to access all 2010 Annual Report forms and the Mechanical Fitting Failure Form. Click on “create account” - 17 -

18 U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration Registering Through the PHMSA Portal Users will need to enter their name, physical mailing address, phone number, email address, whether they are an employee or agent, and then create a user id Then to associate your name to an operator, the OPID and most current ODES PIN must be entered. Once the combination matches, your userid will allow you to submit reports for that Operator ID. This process will be repeated to associate addition OPIDs to your account. - 18 -

19 U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration Logging into the PHMSA Portal Users will receive an email with their userid and temporary password immediately upon registering Users will enter their userid and temporary password to enter the portal; then user will be immediately prompted to change their password. - 19 -

This will display the reports, 2010 Gas Distribution Annual Report(s) or Mechanical Fitting Failure(s), that have been submitted to date. Reports can be exported as Excel or a PDF

Note: Annual Reports expands to list all system types. The only option that will work at this time is Gas Distribution

The only option that will work at this time is Gas Distribution. If you need to submit a Transmission Annual Report that option will be available at the beginning of May.

26 U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration Creating a Gas Distribution Annual Report - 26 - In an effort to reduce the reporting burden, the 2010 Gas Distribution Annual Report can be pre-populated with data submitted on the latest 2009 annual report, by selecting option “Create New Report - with data import” and Selecting the applicable State

27 U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration Creating a Gas Distribution Annual Report Continued - 27 - Report will be completed using the Turbo Tax approach, meaning the system will ask questions that complete the form, section by section. Data quality checks will be built in and enforced prior to submission. If information is unknown, a user can Save a draft to complete at another time.

Unless you have received an exception from PHMSA, to report via paper, all reports MUST be submitted online per the One Rule. All 2010 Annual Reports and Mechanical Fitting Failures will be submitted through the PHMSA Portal. All other forms incident/accident and 2009 and prior Annual Reports will still be submitted using OPID and ODES PIN in via the ODES application - 28 -
