Mission Statement & Goals Our mission is to serve the residents of Mecosta County by providing the highest quality diabetic care through screening, education, prevention, and intervention. Organizational Goals Reduce incidences of diabetes related hospitalizations by 10 percent in Mecosta County by 2020. Reduce the rate of new diabetic cases by 5 percent in Mecosta County by 2020. Initiate a monthly revolving diabetes clinic in Mecosta County by 2015.

Objectives 1. Diabetes Wellness To ensure all diabetics in the county have glucose monitors and know how to use them. Provide monthly wellness visits that include A1c glucose testing, physical examinations, and addressing concerns. 2. Education Monthly diabetic screening clinics. Monthly health and fitness programs. Create and educational website catering to Mecosta County.
