CA Open Enrollment 3/1/11-3/30/11 Final For Plans No Longer Available for Sale as of 9/23/10

2 Overview In order to comply with PPACA, Anthem discontinued offering its existing Individual plans effective 9/23/10. Anthem was required to begin selling PPACA compliant Individual plans effective 9/23/10. Because our plans sold prior to 9/23/10 are closed to new sales, a one-time offer to change to an open PPACA compliant plan offered by Anthem during a 30 day Open Enrollment period. This will be March 1 st -30 th.

3 Additional Info… No medical underwriting will be required, changes may pierce grandfathered status depending on the type of plan movement. Anthem Blue Cross Life and Health Insurance Company insureds on policies regulated by the CDI will be able to choose any open CDI or DMHC policy/plan. Anthem Blue Cross members on plans regulated by the DMHC will be able to choose any open DMHC plan. All plan change requests will be given a 4/1/11 effective date.

4 Why Open Enrollment? 1. California regulatory requirement (CDI and DMHC members) Anthem does not believe the closed block requirements apply to these plans, but is nonetheless providing the option to members. 2. The Serensca Settlement (CDI members only) All members of the Serensca class are to be given a one-time opportunity to move to any open plan at the time their plan is closed and at the time of a rate action through 2013. Serensca class members can select any open CDI or DMHC policy/plan.

5 PPO Share Plans Not Part of the March Open Enrollment The following PPO Share plans were closed in September, 2009. These plans are excluded from the March Open Enrollment because they were closed prior to 9/23/10. 6 ChangeMyCoverage.Com Changemycoverage.com will be temporarily re-purposed from 3/1 to 3/30 to support quoting for March open enrollment as follows: CDI members will see open CDI and DMHC policies/plans ; DMHC members will see open DMHC plans only Special messaging placed on the landing pages to ensure that only members enrolled before 9/23/2010 are eligible Instructions to be added for members to download and return a special Plan Options Change form (no online change submissions) Member’s current policy must be paid to 4/1/11 Member will be allowed to make changes over the phone Site will have language about grandfather status and potential loss due to change of coverage Note: DMHC rate & benefit changes effective 5/1 will NOT be reflected in member’s current coverage on the site.
