7 Employer Voluntary Benefits Group Term Life Insurance Choose from a minimum of $10,000 to a maximum of $500,000 (in $10,000 increments) for both yourself and/or spouse. Rates based on age. Dependent Child(ren): Age 14 days to 6 months:$1,000 Age 6 months to 20 years: ($2,500, $5,000, $7,500, $10,000) (Refer to plan summary for benefit design and rates)

8 Employer Voluntary Benefits Group Accidental Death & Dismemberment Insurance Choose from a minimum of $25,000 to a maximum of $500,000 in $25,000 increments. (Not to exceed 10 times earnings for amounts over $150,000) Employee must be insured in order for Dependents to be covered. (Refer to Plan Summary for plan design and rates)
