2 Types of Dental Licenses in Minnesota (150A.06) –Dentist –Faculty Dentist –Resident Dentist –Specialty Dentist –Internationally Educated Dentist –Dental Hygienist –Registered Dental Assistant –Guest dentist, hygienist, RDA

3 Ways To Obtain Licensure Licensure by Examination (new graduates) Licensure by Credentials (already licensed in another state)

4 Obtaining Licensure by Examination (150A.06 & 3100.1100 to 3100.2000) Application form & Fee ($140) Graduation from accredited U.S. or Canadian dental school Parts I & II of National Board Examination General anesthesia, conscious sedation, nitrous oxide: Complete specific requirements found in M.R. 3100.3600

5 Obtaining Licensure by Examination, cont’d. Pass board approved clinical examination Pass Board’s jurisprudence examination (within past 5 years) Photograph (within past 6 months) Testimonial of good professional character from dental school

6 Obtaining Licensure By Credentials (150A.06 & 3100.1100 to 3100.2000) Graduation from accredited dental school in U.S. or Canada Pass Parts I & II of National Board Exam Must hold current license in another state or Canadian province Must have completed a clinical exam for licensure in another state/province Must have been in active practice for 2000 hours of the previous 36 months in another jurisdiction

Physician’s statement & ophthamologist’s or optometrist’s statement Submit sample of patient records showing treatment rendered by applicant within past 5 years Submit documentation of professional development/ Continuing Dental Education credits for past 60 months

8 Obtaining Licensure By Credentials, cont’d. Must meet with Board’s Licensure & Credentials Committee to respond to questions about knowledge of dental subjects and ability to practice dentistry May apply only ONCE in a 5-year period May appeal Board’s decision if denied licensure

9 License Renewal (3100.1700) License is not granted “for life!” Must be renewed according to Board rules on a biennial basis (based on birth month) Must meet professional development requirements in order to renew Must pay biennial fee to renew

(150A.08 & 3100.6100 – 3100.6400) Grounds for discipline include: Fraud or deception in connection with practicing or in obtaining a license; Conviction…in any court of a felony or gross misdemeanor reasonably related to dental practice; Conviction…in any court of an offence involving moral turpitude; Habitual overindulgence in use of intoxicating liquors Improper prescribing or personal misuse of drugs

Conduct unbecoming a dentist, or conduct not in best interests of public; Gross immorality; Any physical, mental, emotional, or other disability which adversely affects a dentist’s ability to perform dentistry; Disciplinary action taken by another state, territory, country

Failure to maintain adequate safety and sanitary conditions in dental office, in accordance with Board rules; Employing, assisting, or enabling…an unlicensed person to practice dentistry; Failure or refusal to attend, testify, and produce records as directed by Board; “Repeated pattern of conduct”

13 Losing Your License! (150A.08 & 3100.6100 – 3100.6400) Grounds for Discipline, cont’d. Failure to comply with disciplinary order; Knowingly providing false/ misleading information to Board; Aiding suicide or aiding attempted suicide.

14 Defining Disciplinary Actions Conditional license Limited license Suspended license Voluntary surrender of license or Revoked license Unconditional license All of these are classified as PUBLIC documents

15 Other Public Action Agreement for Corrective Action (“ACA”) Public, but technically not considered “disciplinary action” so.. - not reported to National Practitioner Data Bank and - not published on Board’s web site - does not rise to the level where disciplinary action is warranted, but Board believes changes are needed.

16 Reinstatement of License First, don’t give it up! But, if you do, get it back within 5 years! If you lose licensure privileges because of disciplinary action by the Board, the order will include language specific to how you may get it reinstated.
