Coverage available from $10,000 to $50,000. –Insured Children will be limited to $10,000 of coverage. –This policy is guaranteed renewable to age 75.

15 For Agent Use Only – Not for public distribution How The Benefit Is Paid Paid in a lump sum per category directly to insured. Multiple critical illnesses may be paid within a category as long as the maximum benefit for that category is not exceeded. You are eligible for benefits from another category if the date of diagnosis is 180 days from the previous date of diagnosis for a covered Critical Illness. If you should have two or more Critical Illnesses diagnosed on the same day, Cash Advantage will pay the larger of the Critical Illness benefits for the conditions diagnosed.

16 For Agent Use Only – Not for public distribution Benefit Example Sam chooses $10,000 benefit for each category. Sam still has a remaining $10,000 benefit amount for Categories One and Three. SAM Two Years Later: Sam undergoes Coronary Artery Bypass Surgery. Policy pays Sam $2,500 (25%) from Category Two: Heart/Stroke Conditions. Seven Months After Surgery: Sam has a Heart Attack. Policy pays Sam $7,500 (75%) from the remaining balance of Category Two: Heart/Stroke Conditions.

Hospital Indemnity Benefit –Daily cash benefit for hospital confinement as an inpatient with maximum of 30 days. –Pays 2 times the amount if in Intensive Care Unit –Daily benefit amounts between $100 - $1000 (in $100 increments) Rider availability varies by state

Accidental Death & Dismemberment Benefit –We will pay a cash benefit for Accidental Death or Dismemberment due to injuries received in a covered accident or within 90 days after the date of the covered accident. Benefit amounts are $25k, $50k, $75k and $100k. Rider availability varies by state

Return of Premium Benefit –Benefit will be paid on the named insured's 75th birthday or after coverage has remained in force for 20 consecutive years. –Benefit is equal to premium paid (based on the rate at issue) less claims paid –Rider terminates when the total amount of claims paid exceeds the amount of this benefit. Rider availability varies by state

Cash Value (LA, ND, TN & TX only) –If policy terminates after the fifth consecutive year with the rider inforce, the benefit payable will be the original premium multiplied by the applicable percentage factor (less any claims paid). –Benefit is equal to premium paid (based on the rate at issue) less claims paid multiplied by the applicable percentage as shown in the Table of Cash Value Percentages within the rider. –Payable only once –Automatically pays out and terminates on 20 th rider anniversary date. Rates –Coverage Type – Individual, One Parent, or Family –Age Use the Issue Age of the oldest applicant –Gender –Tobacco Use Family applicants must use tobacco rates if one member has used tobacco in the last five years. –Rates include a non-commissionable Annual Policy Fee of $48. –Minimum Premium of $20.00 per application.
